Complete the 2023 NYC School Survey!

  • Parents can take the survey online via computer or mobile device at https://surveys.panoramaed.com/nycdoe/login 
  • Your access code is “f” followed by your child’s 9-digit Student ID (OSIS). 
  • Please note, the “f” must be lowercase.
  • Families can find their child’s OSIS on their report card or NYCSA account. 
  • You must complete a survey for each child you have enrolled at a DOE school.
  • The Survey will close on March 31.
